If you received a letter from the Canada Revenue Agency about your eligibility for the Canadian Dental Care Plan, please complete the first page of the form and submit it to the Teachers’ Pension Plan through the Message Centre in My Account. You can also mail the form to Teachers’ Pension Plan, PO Box 9460, Victoria BC, V9W 9V8 or fax it to 250 953-0431. The plan will complete the form and return it to you.
